In preparation for your visit, please bring a valid identification card such as a state issued driver’s license, your insurance card (s), and a credit card to keep on file. Arrive 20 minutes prior to your appointment to ensure enough time to fill out the patient paperwork. You can also access your online portal to fill out the intake information prior to your visit. Minors must be accompanied by their legal guardian for their first visit. If you wish for your minor to be seen without you after their first visit, please sign the consent form at the first visit authorizing us to see the minor without your presence.

We can still see and care for your dermatologic needs, even if we are out-of-network. We encourage all patients (whether we are in-network or out-of-network) to confirm your plan benefits with your insurance company. Prior to your appointment, we can provide you with an estimate of the CPT codes and charges. You will pay for the estimated charges at your visit and you will be provided with a medical superbill. You will be able to submit the receipt for your visit to your insurance for reimbursement via your out of network plan, depending on your benefits.

Lone Star Dermatology has been working extensively to be in-network with various insurance companies. If you are like us and find it inconvenient that we are not in-network with your insurance company, we encourage you to submit a Network Adequacy Complaint to the Texas Department of Insurance and request that Dr Harper and Lone Star Dermatology be added to your insurance’s network panel.

Please confirm with your insurance company prior to your visit whether a referral is required or not.
